Conditions of Enrolment

I understand that whilst the MSID teacher(s) are suitably experienced and qualified, and that all reasonable care is taken to ensure a safe dance environment; there is always a degree of risk (of strain or bodily injury) involved. Each student participates at their own risk and the teacher(s) of MSID are not liable for any injuries sustained during dance classes.

I hereby give my permission for the teacher(s) of MSID to seek medical attention for my child should the need arise. (OR: I acknowledge that if I DO NOT give my permission for the teacher(s) of MSID to seek medical attention for my child should the need arise, I will personally email munroschool@hotmail.com advising of this requirement).

I give permission for MSID to use my child’s first name(s), picture and/or videos in media, social media (Facebook, Instagram), advertising and/or promotional materials for MSID and such use shall solely be for this purpose. (OR: I acknowledge that if I do not give permission, I will personally email munroschool@hotmail.com advising of this requirement).

I will ensure that both my child and I have read and understood the MSID Code of Conduct and MSID Rules (available in the MSID Mobile App or via email upon request) and will help to enforce and promote them with positivity.
